Sometimes. It depends on the medical limit, whether you had to pay for the trip with the card, and how long you're away. Cards often cap medical cover well below what a hospital stay costs in the US.

What does "only if you paid with the card" mean?

Some cards only cover you if the flights or package were charged to that card. If you paid another way, the cover never starts.

How much medical cover do I need?

There's no single number. Rules of thumb used by many travelers: $500,000 or more for the US and Canada, and $100,000 to $250,000 for most other places.
